The objectives of this visit are for us to become acquainted with one another, to review your health history and the health history of your family and go over the clinic policies, regular and follow-Up Appointments.
Once we have received the necessary information, we will email you the date and time for a Meet and and Greet appointment. This maybe in-clinic or by phone, depending upon the physician’s preference for this type of appointment.
During this conversation, you will have the opportunity to discuss your healthcare needs, ask any questions you may have, and get to know our new physician. In some cases, if your medical history is more complex, a follow-up, in-person visit may be required to assist in building rapport and the establishment of a therapeutic patient-doctor relationship.
- All Adults must be present for this appointment
- Individuals 14 years and older are considered an adult and must speak to the physician directly.
- Children under 14 years do not need to be present for this appointment, but the physician will discuss their medical and social history with the parent.

Please keep in mind that this Meet and Greet interview is not a formal acceptance as a patient at Fraser Medical Clinic. It is an opportunity for both parties to determine if our clinic is the right fit for your healthcare needs. The doctor reserves the right to decline to attach you to the practice.

Your meet-and-greet phone interview is an essential step in this process, allowing us to get to know you and understand your healthcare requirements better. If you are unable to attend your appointment and do not notify us at least 2 business days in advance, we regret that we will be unable to accept you as a patient.
